Updated Patient Forms (for the new office) are required, even if you are a previous patient of Dr. Ha's  from her Cerritos office. Please print the Patient Registration Form and Patient Health History Form, fill them out completely and bring them with you to your first appointment.  This will save you unnecessary waiting before your visit with Dr. Ha.
